Americas

For portability and Letter of Agency (LOA) forms, refer to the details below:

Country	Contracts/Forms
U.S.	Choose between 2 LOA forms: Individual LOA:Letter of Agency for Preferred Carrier Change Use this form for individual port requests (with no repeats subsequently). Customer must complete and sign this form each time. Blanket LOA:Letter of Agency for Preferred Carrier Change Blanket Approval Customer completes and signs this form one time and anytime there is a subsequent port request within a 12-month period. Attach a copy of the previously signed blanket LOA to a new Billing Telephone Number (BTN) Selection Form for each port order. Either add Telephone Numbers (TNs) on the BTN Selection Form or attach a separate document to the BTN Selection Form with the TN and check the box on the BTN Selection Form that states: "separate page for additional TN." The BTN Selection Form is the second page of the Letter of Agency for Preferred Carrier Change Blanket Approval. A new Blanket Approval must be signed each year.
Canada	VoIP Canada Customer Letter of Authorization Note: Customer signature required to authorize Allstream to act on the customer's behalf with other service providers to transfer any customer services.
